Prepare your notes for a vet visit

Make a short, useful summary without trying to diagnose the problem yourself.

Write down what you noticed

Describe the change in your own words and when you first noticed it. Separate observations from guesses. Bring any photos or notes you want your veterinarian to review.

Bring the existing information

Gather recent records and the names of any products or medicines your pet is currently receiving. Let the veterinarian interpret them and explain the next steps.

Leave with a clear record

Ask for written instructions and clarify anything you do not understand. Save the visit date, documents and agreed follow-up. If your pet needs urgent help, contact a veterinarian directly rather than waiting to finish organising notes.
